2. I choose this photo, and flipped it 90 degrees, so it would fit better. Click the photos below for a larger version.   3. Open up a blank new image, the same size as your picture, or bigger. 4. Using the text tool, type your name, in black. Here is mine so far:  5. Now go to the picture of yourself, and press Control A + Control C. This highlights it all, and then copies it. 6. Switch back to the image with your name. Press Control V. This pastes your picture on top of the text. 7. On the layers palette, set the mode to 'screen.'  8. Now using the pointer tool, position the picture wherever you like. 9. You're done! :) My final result came out like below. Click it to view the full size.
